Request links: create from the new password form, with configurable defaults

Right now, a password request link can only be created from an existing password record, so the technician has to create and save a password (often with a throwaway value) before they can request the real credential. We’d like a “request password” option directly on the new password form: name the record, choose the fields to request, and generate the link in one pass.

It would also help to set request link defaults (expiration, requested fields, message, notification email) in admin settings, and to create and send the link in one action instead of creating it and then sending it separately.
