I feel like we need multiple additional field options: email, address, ‘condefedential' field that does not check for compromises (have a ‘Locations’ asset where we are trying to put in lockbox codes but they all say compromised 909 times, etc.), if then fields (e.g. pulldown that has 3 options, which then spawns multiple other fields based on the option selected in the above option), multi-select options (e.g. Provider X provides services 1, 2, & 4, but not 3, & 5)
